Reminder – you start at Step 1 – Determine why you are creating a customer survey:
• What do you want to know?
• Who do you want to know if from?
• What are you going to do with the information once you get it?

Start the questionnaire with a couple of questions about them – what is important to them, what do they value, what are they concerned about.
Then ask a couple of questions about the product, service or experience
Then a couple of questions asking for improvement ideas, what they value most about the company.
